To apply for this program, please look at the requirements and then download the application.

To fill out a PRC Application, please download the file  below, print it out, and fill in the information.  If applying for shelter expenses, download the landlord statement below. When finished, please return to the  Job and Family Services office at 95 East Main Street, Carrollton, OH 44615.
